This self-management education workshop originally developed at Stanford University Patient Education Research Center and approved by the CDC, can help with a variety of chronic or ongoing health problems like arthritis, diabetes, heart disease, and lung disease.

All graduates will receive a toolkit and the book: "Living a Healthy Life with Chronic Conditions" (a valuable resource for each participant as they travel on the path to better health and towards an improved quality of living).
